Job summary

CommuniCare Rehab in Berkeley Springs is seeking a Physical Therapist (PT) for a Full Time position at the Healthcare Center, where therapists are in‑house team members.

The role involves evaluating residents, developing and implementing treatment plans, and collaborating with the rehab team to ensure quality care. A Bachelor's, Master's, or Doctorate in PT and a valid or pending state license are required.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Current or pending State Physical Therapy license.

Responsibilities

• Evaluate and review patient charts and develop a treatment plan.
• Implement treatment plans and supervise PTAs as needed.
• Lead interdisciplinary meetings and share clinical findings with rehab team.
• Work at multiple centers to serve diverse patient needs.

Qualifications:

• A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.




• Hold a valid or pending State Physical Therapy License

Job Responsibilities include, but are not limited to:

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.




• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.




• Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
