Care IV Home Health in Little Rock, AR is seeking a full-time Physical Therapist to join our team. You will provide in-home physical therapy services in patients' homes or assisted living facilities, developing individualized care plans and tracking progress for each patient. Active Arkansas PT license required.

Care IV Home Health is a locally owned and operated home health agency in Little Rock, AR, seeking a full-time Physical Therapist to join our esteemed team. Requirements:

• Active Arkansas Physical Therapy license or registered in the state of Arkansas.
• A minimum of three professional references on the application or upon initial contact.
• Home Health experience is a plus but not required.

Duties may include, but are not necessarily limited to:

• Visit patients within our Little Rock service area to provide care in their homes or assisted living facilities.
• Develop and execute personalized physical therapy care plans for each patient.
• Record patient progress, including admissions, discharges, OASIS assessments, and regular updates to the Director.
• Complete any other assigned tasks.
