Job summary

Flagstar Rehab is actively seeking Licensed Physical Therapists (PT) to join our skilled nursing facility teams across NYC and surrounding areas. You will perform evaluations, develop individualized treatment plans, and work within a multi-disciplinary team to improve patient function and cognition.

We offer Direct Employment or Independent Contractor options, with competitive pay ($46–$48 hourly W-2; $57–$60 hourly 1099), weekly pay, paid orientation, sick leave, insurance options, commuter

Qualifications

• Current NYS PT License and Registration.
• CPR Certification.
• EMR Software proficiency.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting with diverse diagnoses (e.g., TBI, CVA, Stroke, Parkinson's, MS, cancer, orthopedic surgery).
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
