Amergis Healthcare Staffing seeks a Travel Physical Therapist for assignments in Santa Rosa, CA. Responsibilities include performing patient evaluations and providing therapy per plan of care.

The role requires PT licensure in California, graduation from an accredited PT program, and CPR certification; a minimum of 1 year of experience is preferred. Benefits include competitive pay and comprehensive health coverage.

• One year of prior professional Physical Therapy experience preferred.
• CPR certification if applicable.
• TB Questionnaire, PPD or chest x-ray if applicable.
• Health certificate per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsabilidades

• Perform patient evaluations and document therapy services in accordance with the plan of care.
• Provide therapy services per physician orders when applicable.
• Maintain accurate records and adhere to facility policies.

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

Minimum Requirements

• Current licensure as a Physical Therapist in the state ofpractice


• Graduate of an accredited school of Physical Therapy


• One (1) year of prior professional Physical Therapyexperience preferred


• Current CPR if applicable
