Job summary

Amergis Healthcare Staffing is seeking a Travel Physical Therapist to evaluate patients and deliver and document physical therapy services in accordance with the plan of care and physician orders where applicable.

The role requires active PT licensure in the practice state, CPR if required, and compliance with applicable health and regulatory standards. Travel assignments may involve staying at different facilities and schedules around 40 hours per week.

Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• One year of prior professional Physical Therapy experience preferred.
• CPR if applicable
• TB Questionnaire or chest x-ray if applicable
• Health certificate per contract/state regulation.

Responsibilities

• Perform patient evaluations and provide therapy services as per the plan of care.
• Document therapy services and align with physician orders when applicable.

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician’s orders when applicable.

Minimum Requirements

• Current licensure as a Physical Therapist in the state of practice
• One (1) year of prior professional Physical Therapy experience preferred
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• TB Questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
