Job summary

Shady Grove Center for Nursing & Rehabilitation in Rockville, MD is seeking a licensed Physical Therapist (PT) for a full-time role. You will provide direct patient care, assess functional abilities, and develop treatment plans aimed at rehabilitation goals for our residents in long-term care settings.

Collaborating with interdisciplinary teams, you will document patient progress, educate families, and contribute to a compassionate work environment.

Qualifications

• Must hold a valid professional license to practice as a Physical Therapist.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals based on patient capacity and tolerance under physician direction.
• Document patient condition and educate patients and families in appropriate PT methods.
• Collaborate with all disciplines to deliver patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.

• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.

• Collaborates with all disciplines to provide best patient centered treatment and care.

Requirements/Qualifications:

• Must have a valid professional license and registration for state of practice.

• Experience with long-term care and/or geriatric care is preferred
