Job summary

Medical Facilities Of America, Inc. at Hyattsville Nursing & Rehab Center is seeking a Physical Therapist (PT) for a full-time role.

You will provide direct patient care, assess medical conditions, and develop rehabilitation plans tailored to residents in a long-term care setting. The position emphasizes collaboration with a multidisciplinary team to deliver patient-centered treatment and support professional growth.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Providing direct patient care to assess medical conditions, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with goals determined according to the patient's capacity and tolerance under the Physician's direction.
• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
