Job summary

Hyattsville Nursing & Rehab Center is seeking a Physical Therapist (PT) for a full-time position in Hyattsville, MD. The therapist will provide direct patient care, assess function, and contribute to rehabilitation plans while working within a collaborative care team.

Eligible candidates should hold an active PT license and have experience in long-term/geriatric care. A $10K retention bonus is offered as part of this role to support ongoing service excellence.

Qualifications

• Must hold a valid PT license/registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Provide direct patient care to assess medical condition and rehabilitation potential.
• Establish and administer treatment plans with physician-guided goals.
• Document patient conditions and educate patients and families in PT methods.
• Collaborate with disciplines to deliver patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.

Requirements/Qualifications:

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred
