Job summary

Rehab at Home Healthcare Services is hiring a Physical Therapist to perform home-based PT evaluations and treatment plans in the Plano, TX area. The successful candidate will hold a current Texas Physical Therapist license and a PT degree from an accredited program, with a valid Texas Driver’s License.

Home health experience and pediatric exposure are a plus but not required. We offer a supportive, mentorship‑driven environment in patients’ homes.

Qualifications

• Current Texas license as a Physical Therapist.
• Degree in Physical Therapy from an accredited program.
• Valid Texas Driver’s License.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsibilities

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient’s needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety and proper techniques.
• Collaborate with physicians, caregivers and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Ensure compliance with applicable clinical, ethical, and legal standards.
• Maintain professional competence and stay current with evidence‑based practices.

Education

Bachelor’s, Master’s, or Doctorate degree in Physical Therapy
