Job summary

Rehab at Home Healthcare Services in Waxahachie, TX is seeking a Physical Therapist to evaluate patients and implement individualized home-based care plans. You will provide therapeutic interventions to improve mobility, strength, balance and independence, and educate patients and caregivers on exercises and safety.

The role requires a current Texas PT license and an accredited PT degree, plus a valid Texas driver’s license. Home health or pediatric experience is preferred but not required.

Qualifications

• Must hold a current Texas Physical Therapist license.
• Graduate from an accredited Physical Therapy program.
• Valid Texas Driver's License.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsibilities

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient's needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety, and proper techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Ensure compliance with applicable clinical, ethical, and legal standards.
• Maintain professional competence and stay current with evidence-based practices.

Physical Therapist Qualifications:
