Rehab at Home Healthcare Services in Texas seeks a dedicated Physical Therapist to deliver home-based care, evaluating and treating patients in their homes and collaborating with a care team. Candidates must hold a Texas PT license and a degree from an accredited program; driver’s license required. Home health and pediatric experience are a plus but not required.

Training and mentorship are provided. This is a full-time role offering opportunities to advance clinically and impact patient

Qualifications

• Current Texas license as a Physical Therapist.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Valid Texas Driver's License.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsabilités

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient's needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety, and proper techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Ensure compliance with applicable clinical, ethical, and legal standards.
• Maintain professional competence and stay current with evidence-based practices.

Formation

Bachelor's, Master's, or Doctorate in Physical Therapy

The ideal candidate for this position will have graduated from an accredited Physical Therapy program and hold a current Texas Physical Therapist license; candidates in the process of pursuing a degree in an accredited physical therapy program are welcome to apply.
