Rehab at Home Healthcare Services is hiring Physical Therapists to deliver home-based therapy across Texas. You will evaluate patients, design and implement plans of care, and guide families on exercises and safety in their homes.

The role emphasizes collaboration with clinicians, ongoing documentation, and keeping up with evidence-based practices to ensure high-quality patient outcomes.

Qualifications

• Must hold current Texas license as a Physical Therapist.
• PT degree from an accredited program (BS/MS/Doctorate).
• Valid Texas driver’s license required.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsabilités

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care.
• Provide therapeutic interventions to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on exercises, home programs, safety, and techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate care.
• Complete accurate and timely documentation to support therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Maintain compliance with clinical, ethical, and legal standards.
• Stay current with evidence-based practices.

Physical Therapist Qualifications:

• Current Texas license as a Physical Therapist
