Rehab at Home Healthcare Services is seeking a Physical Therapist to evaluate and treat patients in home health across the Dallas/Fort Worth area. The role requires a current Texas PT license and a driver’s license; home health experience is preferred but not required, and pediatric experience is a plus.

• Current Texas license as a Physical Therapist.
• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Valid Texas Driver's License.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsibilities

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient's needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety, and proper techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Ensure compliance with applicable clinical, ethical, and legal standards.
• Maintain professional competence and stay current with evidence-based practices.
