Rehab at Home Healthcare Services in Texas is looking for a dedicated Physical Therapist to provide high-quality home-based care. The ideal candidate will hold a current Texas Physical Therapist license and have graduated from an accredited PT program, with openness to applicants pursuing a degree.

The PT will evaluate patients, develop and implement plans of care, deliver interventions to improve mobility and independence, and educate patients and families.

Qualifications

• Current Texas license as a Physical Therapist.
• Degree in Physical Therapy from an accredited program (Bachelor's, Master's, or Doctorate).
• Valid Texas Driver's License.
• Home health experience preferred but not required.
• Pediatric experience preferred but not required.

Responsabilités

• Perform physical therapy evaluations and re-evaluations.
• Develop and implement individualized plans of care based on each patient's needs.
• Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.
• Educate patients and caregivers on therapeutic exercises, home programs, safety, and proper techniques.
• Collaborate with physicians, caregivers, and other healthcare professionals to coordinate patient care.
• Complete accurate and timely documentation to support the delivery of physical therapy services.
• Monitor patient progress and update treatment plans as appropriate.
• Ensure compliance with applicable clinical, ethical, and legal standards.
• Maintain professional competence and stay current with evidence-based practices.
