Job summary

Piedmont Post Acute in Piedmont, SC is seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide skilled therapy designed to improve mobility, strength, function, and independence.

This full-time, on-site role requires SC licensure in good standing, a degree from an accredited Physical Therapy program, and a commitment to documentation, regulatory standards, and collaborative care with

Qualifications

• Degree from an accredited Physical Therapy program.
• Current South Carolina Physical Therapist license in good standing.
• Long-term care or skilled nursing experience preferred.
• Experience with PCC and Casamba preferred.
• Must maintain continuing education and licensure requirements.

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders
• Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Complete accurate and timely documentation
• Participate in care plan meetings and discharge planning
• Educate residents and caregivers on exercises, safety, and mobility strategies
• Maintain compliance with all regulatory and professional standards
• Support positive resident outcomes and quality-of-life goals

Position Summary

• Strong clinical, communication, and organizational skills
• Must maintain all required continuing education and licensure requirements
