Job summary

Piedmont Post Acute in Piedmont, SC is seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to improve mobility, strength, function, and independence.

Must have a degree from an accredited Physical Therapy program and a current South Carolina license. Long-term care experience and familiarity with PCC/Casamba are preferred; weekend availability is required.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current South Carolina Physical Therapist license in good standing.
• Experience in long-term care or skilled nursing preferred.
• Experience with PointClickCare (PCC) and Casamba preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services per physician orders.
• Monitor progress and adjust treatment plans as needed.
• Document accurately and timely; participate in care plan and discharge planning.
