• Educate residents and caregivers on exercises, mobility strategies, and safe movement.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Southgate. This full-time opportunity is a great fit for a therapist who enjoys helping residents rebuild strength, improve mobility, and work toward greater independence through individualized rehabilitation.

What You'll Do

• Evaluate residents and develop personalized physical therapy plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility
• Establish appropriate rehabilitation goals and track resident progress
• Adjust treatment plans based on clinical findings and individual needs
• Educate residents and caregivers on exercises, mobility strategies, and safe movement
