Job summary

Ultimate Therapy Services in Essex, MD is seeking a Physical Therapist (PT) for a full-time on-site role at Autumn Lake at Riverview. You will evaluate residents, develop individualized plans of care, and provide skilled therapy to improve mobility and independence.

You will collaborate with PTAs, OT, SLP, nursing, and other team members, document clinical progress, and educate residents and caregivers on exercises and safety strategies.

Qualifications

• Active Maryland Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical judgment, communication, and teamwork skills.
• New graduate Physical Therapists are welcome to apply.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and functional mobility.
• Establish measurable rehabilitation goals and monitor resident progress.
• Adjust treatment plans based on clinical findings and individual needs.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Riverview. This full-time opportunity is ideal for a therapist who enjoys working directly with residents to improve mobility, restore function, and support greater independence throughout the rehabilitation process.

What You'll Do
