Job summary

Ultimate Therapy Services seeks a Physical Therapist (PT) to join Autumn Lake at Nellas in Elkins, WV. This full-time, on-site role focuses on evaluating residents and delivering individualized therapy to improve mobility and independence.

You will work with a collaborative rehabilitation team, educate residents and caregivers, maintain timely documentation, and pursue ongoing professional development.

Qualifications

• Active West Virginia Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• New graduates are welcome to apply.

Responsibilities

• Complete PT evaluations and develop individualized plans of care.
• Provide skilled treatment focusing on strength, gait, balance, transfers, endurance, and functional mobility.
• Establish measurable rehabilitation goals and track resident progress.
• Adjust treatment approaches based on clinical findings and response to therapy.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Maintain accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary care team.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Nellas. This full-time opportunity offers the chance to provide individualized care while helping residents improve mobility, rebuild strength, and work toward greater functional independence.

What You'll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, gait, balance, transfers, endurance, and functional mobility
