Job summary

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at New Britain in New Britain, CT. This full-time position offers the opportunity to provide individualized care and help residents improve movement and regain independence.

You will perform PT evaluations, develop plans of care, deliver treatments for strength, balance, gait, and mobility, and collaborate with PTAs, OT, SLP, nursing, and others. New graduates are welcome to apply.

Qualifications

• Active Connecticut Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.

Responsibilities

• Complete PT evaluations and develop individualized plans of care.
• Provide skilled treatment focusing on strength, balance, gait, transfers, endurance, and mobility.
• Establish functional goals and monitor resident progress.
• Adjust treatment approaches based on clinical findings and resident response.
• Educate residents and caregivers on exercises, mobility strategies, and safety.
• Maintain accurate and timely clinical documentation.
• Collaborate with PTAs, OT, SLP, nursing, and other team members.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at New Britain. This role offers the opportunity to provide individualized care while helping residents improve movement, rebuild strength, and work toward greater functional independence.

What You'll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment focused on strength, balance, gait, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress throughout treatment
• Adjust treatment approaches based on clinical findings and resident response
• Educate residents and caregivers on exercises, mobility strategies, and safety
• Maintain accurate and timely clinical documentation
• Collaborate with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team
