Job summary

Ultimate Therapy Services is seeking a dedicated Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Vineland in Vineland, NJ. This full-time position offers a competitive hourly rate and benefits, including health coverage and a 401(k).

You will evaluate residents, design personalized plans of care, and work with a multidisciplinary team to restore movement and independence. New graduates are welcome to apply.

Qualifications

• Active New Jersey Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Strong clinical, communication, and interpersonal skills.
• New graduate Physical Therapists are welcome to apply.
• Experience in skilled nursing or post-acute rehab preferred, but not required.

Responsibilities

• Complete physical therapy evaluations and develop individualized plans of care.
• Provide skilled treatment addressing strength, balance, gait, transfers, endurance, and functional mobility.
• Establish rehabilitation goals and track resident progress.
• Adapt treatment approaches based on resident needs and response to therapy.
• Educate residents and caregivers on exercises, mobility strategies, and safe movement techniques.
• Maintain accurate and timely clinical documentation.
• Coordinate care with PTAs, OT, SLP, nursing, and other rehab team members.

Ultimate Therapy is seeking a Physical Therapist (PT) to join the rehabilitation team at Autumn Lake at Vineland. This full-time opportunity is ideal for a therapist who enjoys working with residents to restore movement, build strength, and improve overall function through personalized rehabilitation.

What You’ll Do

• Complete physical therapy evaluations and develop individualized plans of care
• Provide skilled treatment addressing strength, balance, gait, transfers, endurance, and functional mobility
• Establish appropriate rehabilitation goals and track resident progress
• Adapt treatment approaches based on each resident's needs and response to therapy
