Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a compassionate Physical Therapist to join our skilled nursing and rehabilitation team on a PRN schedule. The PT will provide high-quality therapy to residents, improving mobility and independence in a SNF setting.

Required is a Florida PT license with at least 1 year of SNF experience. The role involves evaluation, treatment planning, interdisciplinary collaboration, and staff education to support resident-centered care.

Qualifications

• Current, active Florida Physical Therapist license required.
• Minimum 1 year of Physical Therapy experience in a nursing home/SNF setting preferred.

Responsibilities

• Evaluate residents and develop individualized PT treatment plans and goals.
• Provide skilled PT services focused on mobility, gait, balance, strength and independence.
• Implement treatment programs and document progress accurately.
• Monitor outcomes and modify programs based on resident needs.
• Educate staff and residents on therapeutic exercises and mobility techniques.
• Collaborate with nursing, rehabilitation, physicians, and interdisciplinary teams.
• Participate in care plan meetings and care conferences.
• Coordinate adaptive equipment and modalities as needed.
• Supervise supportive personnel per regulations.
• Maintain clinical documentation in compliance with standards.

Breezy Hills Rehabilitation and Care Center is seeking a compassionate and experienced Physical Therapist (PT) to join our skilled nursing and rehabilitation team. The Physical Therapist will provide high-quality therapy services to residents in a SNF/long-term care setting, helping improve mobility, strength, balance, independence, and overall quality of life.

Key Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans and goals.
• Provide skilled PT services focused on mobility, gait, balance, strength, coordination, endurance, posture, and functional independence.
• Implement treatment programs and document resident progress accurately and timely.
• Monitor functional outcomes and modify therapy programs based on resident needs and progress.
• Provide training and instruction on therapeutic exercises, mobility techniques, and adaptive equipment.
• Collaborate with nursing, rehabilitation, physicians, and interdisciplinary care teams.
• Participate in care plan meetings, resident care conferences, and interdisciplinary team meetings.
• Recommend and coordinate appropriate adaptive equipment, orthotics, prosthetics, and physical modalities.
• Supervise supportive personnel in accordance with state and professional regulations.
• Educate nursing, medical, and other facility staff on appropriate physical therapy techniques.
• Maintain and monitor therapy equipment and report repair or replacement needs.
• Maintain accurate clinical documentation in accordance with facility, state, and professional standards.
• Promote a compassionate, respectful, and resident-centered environment.

Qualifications

• Current, active, and unencumbered Florida Physical Therapist (PT) license required.
• Minimum 1 year of Physical Therapy experience in a nursing home/SNF setting preferred.
• Experience with long-term care, skilled nursing, rehabilitation, gait training, balance training, and functional mobility preferred.
• Strong communication, clinical, organizational, and interpersonal skills.
• Commitment to providing quality, resident-centered care.
