Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a compassionate Physical Therapist to join our SNF team on a PRN basis. You will assess residents, develop individualized plans, and provide skilled PT services to improve mobility, balance, and independence.

The ideal candidate holds an active Florida PT license, has at least 1 year of SNF experience, and collaborates with nursing and medical staff to support resident-centered care.

Qualifications

• Current, active Florida PT license required.
• Minimum 1 year PT experience in nursing home/SNF preferred.
• Experience with gait training, balance training, and functional mobility preferred.
• Strong communication, organizational, and interpersonal skills.
• Commitment to providing quality, resident-centered care.

Responsibilities

• Evaluate residents and develop individualized PT plans.
• Provide PT services focused on mobility, gait, balance, and independence.
• Document resident progress accurately and timely.
• Monitor outcomes and modify therapy programs as needed.
• Educate staff on therapeutic exercises and mobility techniques.
• Collaborate with nursing, rehabilitation, physicians, and care teams.
• Participate in care plan and interdisciplinary meetings.
• Supervise supportive personnel in accordance with regulations.
• Maintain therapy equipment and report repairs.
• Promote a compassionate, resident-centered environment.
