Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, Florida is seeking a Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work under the direction of a licensed Physical Therapist to provide therapy services that help residents improve mobility, restore function, and achieve independence.

The role requires a Florida PTA license and PTA program graduation, with a preferred 1 year of skilled nursing/rehab experience.

Qualifications

  • Current, unrestricted Florida PTA license.
  • Graduate of an accredited PTA program.
  • Minimum one year in SNF/rehab/long-term care preferred.
  • Strong communication, interpersonal and documentation skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Passion for resident-centered care.

Responsibilities

  • Assist the Physical Therapist in implementing individualized treatment plans.
  • Deliver therapeutic exercises, gait training, transfer training and other interventions.
  • Monitor progress and document treatments, responses and outcomes.
  • Communicate progress and changes to the supervising PT and care team.
  • Maintain accurate, timely clinical documentation.
  • Ensure resident safety by following infection control and safety protocols.
  • Maintain a clean, organized therapy department and report equipment issues.
  • Use PPE and adhere to OSHA and facility safety standards.
  • Demonstrate professionalism and excellent resident/customer service.
  • Participate in interdisciplinary meetings and care planning.

Position: Licensed Physical Therapist Assistant (PTA)

Schedule: Full-Time

Pay Rate: $28–$38 per hour (based on experience)

Breezy Hills Rehabilitation and Care Center is seeking a compassionate and motivated Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. In this role, you'll work under the direction of a licensed Physical Therapist to provide exceptional therapy services that help residents improve mobility, restore function, and achieve their highest level of independence.

Key Responsibilities
  • Assist the Physical Therapist in implementing individualized treatment plans designed to improve residents' mobility, strength, balance, and functional independence.
  • Deliver therapeutic exercises, gait training, transfer training, and other treatment interventions as directed by the supervising Physical Therapist.
  • Monitor resident progress and accurately document treatments, responses, and outcomes in accordance with facility policies and regulatory guidelines.
  • Communicate resident progress, concerns, and changes in condition to the supervising Physical Therapist and interdisciplinary care team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Ensure resident safety by following established infection prevention and safety protocols.
  • Maintain a clean, organized, and well-stocked therapy department.
  • Inspect therapy equipment and promptly report malfunctioning or unsafe equipment.
  • Utilize appropriate personal protective equipment (PPE) and adhere to OSHA and facility safety standards.
  • Demonstrate professionalism, compassion, and excellent customer service when interacting with residents, families, and team members.
  • Participate in interdisciplinary meetings and contribute to resident-centered care planning.
Qualifications
  • Current, unrestricted Florida Physical Therapist Assistant (PTA) license.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Minimum one (1) year of experience in a skilled nursing, rehabilitation, or long-term care setting preferred.
  • Strong communication, interpersonal, and documentation skills.
