About the Role

Breezy Hills Rehabilitation and Care Center is seeking a compassionate and experienced PRN Occupational Therapist (OT) to join our rehabilitation team. The OT will provide skilled therapy services to residents in a Skilled Nursing Facility (SNF) setting, helping promote independence, mobility, safety, and quality of life.

Responsibilities

• Provide individualized occupational therapy evaluations and treatment plans.
• Participate in resident assessments, care planning, and discharge planning.
• Deliver skilled OT services consistent with resident needs and physician orders.
• Maintain accurate therapy documentation, progress notes, and medical records.
• Collaborate with nursing, rehabilitation, administration, and other interdisciplinary team members.
• Support quality assurance, regulatory compliance, and best practices in rehabilitation services.
• Communicate effectively with residents, families, and the interdisciplinary care team.

Qualifications

• Active Florida Occupational Therapist (OT) license.
• Minimum 3 years of OT experience in a Skilled Nursing Facility (SNF) preferred.
