Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, Florida is seeking a Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team on a full-time basis. You will work under the supervision of a licensed Physical Therapist to help residents improve mobility, function, and independence.

Requirements include an active Florida PTA license, graduation from an accredited PTA program, and at least 1 year of experience in a skilled nursing or long-term care setting.

Qualifications

• Current Florida PTA license is required.
• Graduate of an accredited PTA program.
• Minimum 1 year of experience in a skilled nursing, rehabilitation, or long-term care setting preferred.
• Strong communication and documentation skills.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Assist the Physical Therapist in implementing individualized treatment plans.
• Deliver therapeutic exercises, gait training, and transfer training as directed.
• Monitor resident progress and document treatments and outcomes.
• Communicate progress and changes to the supervising PT and care team.
• Maintain accurate clinical documentation and ensure safety protocols.
