Job summary

Amergis Healthcare Staffing seeks a licensed Physical Therapist for a travel assignment in Hazle Township, PA. You will perform patient evaluations and provide and document therapy services per the plan of care and physician orders where applicable.

The position offers 40 hours per week, weekly pay of $2,766, and an array of benefits including health, dental, vision, and life insurance plus a 401(k) savings plan. Eligible depending on employment status.

Qualifications

• Current licensure as a Physical Therapist in the state of practice.
• Graduate of an accredited school of Physical Therapy.
• 1 year of prior professional Physical Therapy experience preferred.
• Current CPR if applicable.
• TB Questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform patient evaluations and provide therapy services per the plan of care and physician orders.
• Document therapy services accurately per policy and protocol.

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.
