Job summary

Amergis Healthcare Staffing seeks a licensed Physical Therapist for a travel assignment in Fairfield, CA. You will evaluate patients, develop and implement treatment plans, and document outcomes according to physician orders and the plan of care.

The role offers weekly pay, competitive benefits including health/dental/vision, and a supportive, patient-focused environment. Prior PT experience and CPR may be required by contract; all regulatory requirements must be met.

Qualifications

• Current license as Physical Therapist in the state of practice
• Graduate of an accredited Physical Therapy program
• One year of PT experience preferred
• CPR certification if applicable
• TB questionnaire, PPD or chest X-ray if applicable
• Current health certificate per contract or regulation
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

Responsibilities

• The PT performs patient evaluations and provides therapy services per the plan of care
• Documents interventions and outcomes according to physician orders
• Coordinates with healthcare team to adjust plans of care as needed

Current PT license

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

Minimum Requirements:

• Current licensure as a Physical Therapist in the state ofpractice
• Graduate of an accredited school of Physical Therapy
• One (1) year of prior professional Physical Therapyexperience preferred
• Current CPR if applicable
• TB Questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or stateregulation)
