Job summary

Amergis seeks a licensed Physical Therapist for travel assignments across healthcare facilities in the Chicago area. You will perform patient evaluations, provide and document therapy services per the plan of care and physician orders, and maintain accurate records.

Requires IL licensure, graduation from an accredited PT program, and CPR as applicable; travel requires compliance with federal, state, and local regulations.

Qualifications

• Licensed Physical Therapist in state of practice.
• Graduate of an accredited Physical Therapy program.
• One year of professional PT experience preferred.
• CPR certification if applicable.
• Health certificate as required by contract or regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform patient evaluations and document therapy services per the plan of care.
• Provide therapy services per physician orders when applicable.
• Document and maintain accurate patient records and charting.

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

• Current licensure as a Physical Therapist in the state ofpractice
• Graduate of an accredited school of Physical Therapy
• One (1) year of prior professional Physical Therapyexperience preferred
• Current CPR if applicable
• Current Health certificate (per contract or stateregulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
