Job summary

Pivot Home Health is seeking a Physical Therapist to provide high-quality therapy in home-based and community settings across Colorado. You will work with children and adults, building development and independence through personalized plans delivered in the comfort of patients’ homes.

Qualifications

• Active Colorado Physical Therapist license is required.
• New graduates and relocating clinicians are encouraged to apply.
• Current CPR certification is required.
• Strong communication, organization, and collaboration skills are essential.

Responsibilities

• Deliver physical therapy services in clients' homes and communities.
• Assess and treat pediatric and adult clients across the lifespan.
• Develop and implement individualized home-based care plans.

Physical Therapist (PT) - Pediatric or Adult - Home Health

Flexible caseloads with no minimums or maximums, tailored to your preferences.
Work within your preferred travel radius—no expectation to commute beyond your comfort zone.

Employment Type: full-time or part-time (FT is 20 visits/week)

Compensation: $80-$90 per regular visit

Caseload: Adults and/or pediatrics
