Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our clinician-led team. Responsibilities include evaluating, developing, and implementing individualized treatment plans in coordination with other disciplines, and educating patients and families on home exercise programs and injury prevention.

This PRN role is based at our White Settlement, TX location and offers the opportunity to work with a leadership team that prioritizes professional perspectives and patient outcomes.

Qualifications

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
