Job summary

Nexus at Chicago Heights is seeking a Physical Therapist to help residents regain independence and participate safely in daily activities, while enhancing quality of life. You will develop individualized treatment plans and administer prescribed PT treatments to relieve pain and restore function.

The role emphasizes a patient-centered approach with a focus on long-term care settings and collaborative care within a team of professionals.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.
