Job summary

Nexus Health is seeking a Physical Therapist to develop individualized treatment plans to help residents regain independence and participate in daily activities. We welcome Missouri-licensed therapists and will cover Illinois reciprocity licensing costs.

Key responsibilities include assessing evaluations, formulating treatment plans, creating rehabilitation programs, and administering therapies to relieve pain and restore function. Growth opportunities and comprehensive benefits are offered.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Knowledge of PCC Electronic Documentation System preferred.
