Job summary

Nexus Health in Mascoutah, Illinois is seeking a Physical Therapist to develop individualized treatment plans to help residents regain independence and participate safely in daily activities.

You will assess patient evaluations, create and implement rehabilitation programs, and administer therapies to relieve pain and restore function, within a collaborative team focused on quality care.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Knowledge of PCC Electronic Documentation System preferred.
• Current knowledge of treatment practices.
