Job summary

Creekside Healthcare Center is seeking a Physical Therapist (PT) to evaluate and treat residents within our facility in California. The role includes planning and administering therapy, performing objective tests, and ensuring safe discharge environments in line with a defined care plan.

Candidates should hold a current California PT license; new graduates are welcome and a sign-on bonus is available. Per diem/on-call considerations may apply based on staffing needs.

Qualifications

• California license to practice Physical Therapy required.
• One year of clinical experience preferred; new graduates welcome.
• Degree in Physical Therapy (BSc/BSc, MSc, or DPT) accepted.

Responsibilities

• Evaluates, plans and administers treatment to patients with neuromuscular and musculoskeletal problems.
• Administers and interprets tests and measurements for strength, coordination, ROM, and respiratory/circulatory efficiency.
• Provides direct resident treatment in accordance with a plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments to ensure safe discharge environments.
• Completes required forms in line with policy and regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

• Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
