Job summary

Creekside Healthcare Center is hiring a Physical Therapist (PT) to evaluate and treat residents with neuromuscular and musculoskeletal concerns. Responsibilities include planning and delivering PT services, performing tests and measurements, maintaining patient records, and ensuring safe discharge through home

Qualifications

• Must hold current California Physical Therapy license.
• One year of clinical experience preferred; new graduates welcome.

Responsibilities

• Evaluate, plan and administer treatment for neuromuscular and musculoskeletal issues.
• Administer and interpret tests for muscle strength, coordination, ROM, and respiratory/circulatory function.
• Provide direct patient treatment per established plan of care.
• Maintain confidentiality of patient records.
• Conduct home assessments to ensure safe discharge.
• Complete required forms and documentation per policy and regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.

• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care..
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
