Elara Caring in Mishawaka, IN is seeking a dedicated Physical Therapist to join our home health team and help grow our provider network. You will evaluate patients, develop individualized treatment plans, and deliver therapy services in patients' homes.

You will monitor progress, coordinate with physicians and caregivers, document care, and supervise therapy assistants as needed. Candidates must have an APTA-approved program, a current PT license, and at least one year of healthcare facility

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
