Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home-based care team in Missouri. You will evaluate patients, develop personalized treatment plans, and deliver high-quality PT services in patients' homes.

You’ll collaborate with physicians and caregivers, monitor progress, document care, and supervise staff as needed. A valid PT license in the state and at least one year of healthcare facility experience are required; travel between patient homes is expected.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel to patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
