Job summary

Elara Caring is hiring a Physical Therapist to deliver high-quality care in patients' homes in Rolla, Missouri. You will assess, plan, and implement individualized treatment plans, ensuring timely documentation and collaboration with the care team.

The role requires a valid PT license, 1 year of healthcare facility experience, and dependable transportation for home visits. A driver’s license is essential to travel across assigned locations.

Qualifications

• Graduate of an APTA approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes.
• Ability to frequently lift up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
