Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home health team in Indiana. You will evaluate patients, develop individualized treatment plans, and provide high-quality therapy in the home setting to improve mobility and function.

You will monitor progress, adjust plans as needed, collaborate with the care team, document thoroughly, and supervise aides as appropriate. APTA-approved education and active PT license are required, plus driving to patient locations.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver’s license or reliable transportation to travel between patient homes and locations.
• Ability to lift, push, pull, and reposition patients up to 50 pounds.

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
