Job summary

Elara Caring is seeking a dedicated Physical Therapist to deliver exceptional home-based care. You will evaluate patients, design personalized treatment plans, and provide in-home physical therapy while monitoring progress and adjusting care as needed.

Collaborate with physicians and care teams; document care accurately; supervise assistants when required. Travel between patient homes with reliable transportation and maintain an unrestricted PT license.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Ability to travel between patient homes with reliable transportation.

As a Physical Therapist, you’ll contribute to our success in the following ways:

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program

• Current, unrestricted PT license valid for the state of work

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
