Job summary

Nexushm in Belleville, IL seeks a licensed Physical Therapist to help residents regain independence and safely participate in daily activities.

You will assess evaluations, design rehab plans, and administer treatments to relieve pain and improve function. A valid IL PT license and SNF/LTC experience are preferred; knowledge of PCC documentation is a plus.

Qualifications

• Active Illinois Physical Therapy license is required.
• SNF/Long-term care experience preferred.
• Knowledge of current treatment practices.
• Familiarity with PCC Electronic Documentation System is a plus.

Responsibilities
