Job summary

Nexushm is seeking a Physical Therapist for our Illinois facility. You will evaluate residents, develop and implement individualized treatment plans, and coordinate rehabilitation to promote independence and safety in daily activities.

Ideal candidates hold an active Illinois PT license, have experience in long-term care settings, and stay current with best PT practices and documentation systems.

Qualifications

• Active Illinois license in Physical Therapy.
• SNF/long-term care experience preferred.
• Knowledge of current PT practices.
• Familiarity with PCC Electronic Documentation System preferred.

Responsibilities

• Assess and interpret patient evaluations and test results.
• Develop treatment plans using varied techniques.
• Create personalized rehabilitation programs for patients.
• Administer prescribed physical therapy to relieve pain and restore function.
