Job summary

Nexushm is seeking a licensed Physical Therapist to develop individualized treatment plans for residents, helping them regain independence and safely participate in daily activities.

You will assess evaluations, create rehab programs, and administer prescribed therapies to relieve pain and restore function. Illinois licensure and LTC experience are preferred.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Assess patient evaluations and test results.
• Develop treatment plans using various techniques.
• Create personalized rehabilitation programs for patients.
