Job summary

Linwood Meadows Care Center in Visalia, CA seeks a Per Diem Physical Therapist (PT) to join our rehab team. The PT will evaluate residents, create treatments to improve mobility and independence, and collaborate with staff to meet goals. Requirements include Doctorate or Master's in Physical Therapy and a California license in good standing.

Casamba and PointClickCare experience are a plus. Compensation ranges from $60 to $65 per hour (BOE), with a flexible per diem schedule and supportive team

Qualifications

• Doctorate or Master's Degree in Physical Therapy from an accredited program.
• Licensed Physical Therapist in California.
• Excellent communication and patient-centered approach.
• Experience with Casamba or PointClickCare preferred.

Responsibilities

• Evaluate residents and determine PT needs and rehab potential.
• Develop, implement, and adjust individualized treatment plans.
• Provide gait training, transfers, balance, strengthening, mobility, and pain management.
• Communicate progress with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interprofessional conferences.
• Complete timely, accurate, compliant documentation.
• Educate residents and caregivers on exercises and fall prevention.
• Recommend assistive devices and durable medical equipment.
• Supervise PTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and infection control.

General Purpose

The Physical Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions designed to improve mobility, strength, balance, endurance, and functional independence.

Essential Duties

• Evaluate residents and determine physical therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on exercises, mobility safety, and fall prevention
• Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs
• Supervise PTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

• Education: Doctorate or Master's Degree in Physical Therapy from an accredited program required
• License: Current California Physical Therapist license in good standing
• Preferred Skills: Experience with Casamba and PointClickCare Strong communication and interpersonal skills Excellent clinical judgment and problem-solving abilities Passion for patient-centered care and functional outcomes
