Job summary

PACS Group is seeking a Licensed Physical Therapist for per diem coverage at Arvin Post Acute - Arvin, CA. Flexible hours, per diem schedule, and a competitive hourly pay range of $65-$75/hour in a skilled nursing facility setting.

Responsibilities include evaluations, care plan development, 1:1 therapy delivery, patient education, team collaboration, and accurate documentation. Prior SNF or geriatric rehab experience is preferred and CPR certification is expected.

Qualifications

• CA Licensed Physical Therapist required.
• Graduate of an accredited PT program.
• CPR certified (or willing to obtain).
• SNF or geriatric rehab experience preferred.

Responsibilities

• Complete evaluations and develop care plans.
• Deliver 1:1 therapy per plan of care.
• Educate patients and caregivers on mobility and home exercise programs.
• Participate in care team meetings and discharge planning.
• Maintain accurate documentation and billing records.
