Job summary

Linwood Meadows Care Center in Visalia, CA is seeking an Occupational Therapist (OT) for per diem arrangements to support residents in rehabilitation and daily living activities. You will evaluate residents, develop individualized treatment plans, provide therapeutic interventions focused on ADLs, mobility, balance, cognition, and safety, and communicate progress with physicians and families.

Qualifications

• Bachelor's Degree in Occupational Therapy required; Master's or Doctorate preferred.
• California OT license in good standing required.
• Casamba and PointClickCare experience preferred.
• Strong communication and interpersonal skills required.
• Excellent clinical judgment and problem-solving abilities expected.

Responsibilities

• Evaluate residents and determine OT needs and rehabilitation potential.
• Develop, implement, and modify individualized treatment plans.
• Provide therapeutic interventions for ADLs, mobility, balance, cognition, and safety.
• Communicate progress with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interdisciplinary team conferences.
• Complete timely, accurate, and compliant documentation.
• Educate residents and caregivers on home programs and fall prevention.
• Recommend adaptive equipment to promote independence.
• Supervise COTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and follow infection control procedures.

General Purpose

The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions to improve independence with daily living skills, mobility, safety, and overall functional performance.

Essential Duties

• Evaluate residents and determine occupational therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide therapeutic interventions focused on ADLs, strength, coordination, balance, cognition, and safety
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention
• Recommend adaptive equipment and assistive devices to promote independence
• Supervise COTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

• Education: Bachelor's Degree in Occupational Therapy required, Master's or Doctorate preferred
• License: Current California Occupational Therapy license in good standing
• Preferred Skills: Experience with Casamba and PointClickCare
• Strong communication and interpersonal skills
• Excellent clinical judgment and problem-solving abilities
• Passion for patient-centered care and functional outcomes
