Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Per Diem Physical Therapist (PT) to join our rehab team. You will evaluate residents, develop individualized plans, and provide mobility-focused therapy to improve independence.

We offer a per diem schedule with competitive pay of $60–$65/hour BOE and opportunities for ongoing training and development in a collaborative, supportive environment.

Qualifications

• Doctorate or Master's in Physical Therapy from an accredited program.
• Current California Physical Therapist license in good standing.
• Experience with Casamba and PointClickCare.
• Strong communication and interpersonal skills.
• Excellent clinical judgment and problem-solving abilities.

Responsibilities

• Evaluate residents and determine PT needs.
• Develop, implement, and modify individualized treatment plans.
• Provide gait training, transfers, balance, strengthening, mobility, and pain management.
• Communicate progress with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interdisciplinary teams.
• Complete timely, accurate, and compliant documentation.
• Educate residents and caregivers on exercises and fall prevention.
• Supervise PTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and infection control procedures.

General Purpose

The Physical Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions designed to improve mobility, strength, balance, endurance, and functional independence.

Essential Duties

• Evaluate residents and determine physical therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide treatment focused on gait training, transfers, balance, strengthening, mobility, and pain management
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on exercises, mobility safety, and fall prevention
• Recommend assistive devices and durable medical equipment such as walkers, canes, and wheelchairs
• Supervise PTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

Education:

• Doctorate or Master's Degree in Physical Therapy from an accredited program required

License:

• Current California Physical Therapist license in good standing

Preferred Skills:

• Experience with Casamba and PointClickCare
• Strong communication and interpersonal skills
• Excellent clinical judgment and problem-solving abilities
• Passion for patient-centered care and functional outcomes
