Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Per Diem Physical Therapist to join our rehab team. You will evaluate residents, develop individualized treatment plans, and provide interventions to improve mobility, strength, balance, and independence.

This role offers a per diem schedule with competitive pay at $60–$65 per hour, ongoing training, and a collaborative team environment focused on patient-centered care and meaningful outcomes.

Qualifications

• Doctorate or Master’s in Physical Therapy from an accredited program required.
• Current California Physical Therapist license in good standing.

Responsibilities

• Evaluate residents and determine PT needs and rehab potential.
• Develop, implement, and modify individualized treatment plans.
• Provide gait, transfers, balance, strengthening, mobility, and pain‑management therapies.
• Communicate progress with physicians, nursing staff, residents, and families.
• Participate in care plan meetings and interdisciplinary conferences.
• Educate residents and caregivers on exercises, mobility safety, and fall prevention.
• Supervise PTAs and therapy aides as appropriate; maintain safe environment and infection control.
• Document timely, accurate, and compliant records.
