Job summary

  • Treat residents according to physician-approved plans of care; supervise PTAs and aides.
  • Educate staff and participate in care conferences and rehab meetings.
  • Complete evaluations and documentation per regulatory and payer requirements.

Job description

Physical Therapist (PT) – Full-Time

Location: Modesto, CA

Pay Range: $55.00 – $60.00 per hour (BOE)

Status: Full-Time

Setting: Skilled Nursing Facility (SNF)

Garden City Health Care Center is seeking a compassionate and skilled Physical Therapist (PT) to join our dedicated rehabilitation team. We support our residents in achieving their highest level of independence and quality of life through patient-centered, evidence-based care. General Purpose

The Physical Therapist evaluates and treats residents, communicates with families, physicians, and interdisciplinary team members, and maintains accurate clinical documentation. The PT also provides education and in-services to facility staff to support optimal resident outcomes.

Essential Duties and Responsibilities
  • Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately
  • Develop individualized treatment plans and obtain physician approval
  • Treat residents according to physician-approved plans of care
  • Supervise Physical Therapy Assistants (PTAs), aides, and students in compliance with state practice act
  • Assist nursing staff with training of Restorative Aides
  • Communicate resident progress, challenges, and plans with rehab and interdisciplinary teams
  • Participate in patient care conferences, utilization review, and rehab meetings as needed
  • Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements.
  • Educate residents, families, and caregivers on treatment plans, maintenance programs, and discharge preparation
  • Participate in discharge planning and care coordination
  • Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence
  • Report equipment issues to ensure safe and effective operation
  • Maintain positive, professional relationships with residents, families, staff, and colleagues
Supervisory Responsibilities
  • Supervise Physical Therapy Assistants, aides, and students
Qualifications
  • Education & Experience Bachelor’s degree in Physical Therapy required
  • Master’s or Doctorate degree in Physical Therapy preferred
  • Skilled nursing or geriatric experience preferred
Licensure & Certifications
  • Current, active California Physical Therapist license in good standing
  • Must meet all continuing education requirements per state regulations
Skills
  • Proficient in electronic documentation systems (Casamba and PointClickCare preferred)
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently and collaboratively
Physical Demands
  • Frequent standing, walking, reaching, pushing, and pulling
  • Ability to exert up to 50 pounds frequently and 100 pounds occasionally
  • Occasional climbing, balancing, stooping, kneeling, crouching, or crawling
  • Visual abilities include close, distance, color, peripheral, and depth perception
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions
